What is a Limited Health Insurance Plan?
These plans provide a limited amount of coverage, unlike major medical plans. They specify a maximum number of visits per type of service, with a specific dollar amount per service.
Can I enroll whenever I want to?
No, you must enroll within 30 days of your first assignment. You can also enroll during Open Enrollment periods or when you have a Qualifying Life Event.
What is Open Enrollment?
Open Enrollment is a period when an employee can enroll in health plans or make changes to his/her insurance coverage with no restrictions. Open Enrollment is offered during the month of September only.
What is a Qualifying Life Event?
A Qualifying Life Event is defined as a change in your status due to one of the following: 1) marriage or divorce, 2) birth or adoption of a child or children, 3) loss of insurance coverage involuntarily, 4) death of an immediate family member, 5) Medicare entitlement, 6) employer bankruptcy, 7) loss of dependent status, or 8) loss of prior coverage.
Do I need to provide documentation of a Qualifying Life Event?
Yes, proof must be provided within 30 days of the event.
How do I enroll in a Rx relief® Benefit Plan?
You can enroll in one of two ways: 1) calling Customer Service at 888-208-1998, or 2) going to www.paimembers.com
How do I file a claim?
Complete a Claim Form, attach a copy of the bill, and mail it back to the address indicated on the form. Reimbursement can take from 2-4 weeks.
If I go off assignment do I lose my health coverage?
Not necessarily. You have two options: 1) while you are off assignment you can send in your premium payment directly to the administrator using a Missed Premium Direct Payment Form, or 2) you can choose not to pay your premium for up to six weeks and will still be considered enrolled (but not covered) for benefits during the week(s) that a premium has not been paid.
What happens after six weeks of being off assignment?
You will be offered COBRA coverage and will need to pay the monthly premium if you wish to retain coverage.
What is COBRA coverage?
Mandated by law, COBRA allows employees to continue healthcare coverage that would otherwise be terminated upon job loss.
Is COBRA coverage more expensive?
Yes, there is approximately a 2% increase in the cost.
How long can I be on COBRA?
Depending on the state you live in and your circumstances, COBRA benefits may extend from 18 to 36 months.
