Part-Time On-Call Infusion Pharmacist

 

Job Responsibilities

Pharmacist clinical activities include therapy assessment, care planning and medication reconciliation as they pertain to individual patients and the appropriate monitoring for effective care and positive patient outcome. This is a dynamic work environment and requires flexibility to meet business needs and adaptability to frequent changes.   • Reviews medication orders and evaluates appropriateness and completeness of orders with consideration given to the special needs and behaviors of the specific age group served. Verifies all prescriptions not directly received from the prescriber or physician’s agent
  • Provide oversight of the safety; appropriateness, and effectiveness of the pharmaceutical care provide to patients (consumers) as it relates to specialty products dispensed
  • Assesses the patient at the initiation of drug therapy, develops a patient care plan and drug monitoring regimen and monitors the patient according to the care plan throughout the course of therapy
  • Communicates with the patient, physician, nursing staff, other pharmacists and other members of the health care team regarding patient status, response to therapy and clinical objectives to ensure clinical outcomes are met
  • When required, compounds intravenous and specialty pharmaceuticals in an aseptic fashion according to established State Board of Pharmacy, Federal, USP 797 or 800 sterile guidelines and company policy. Knowledgeable of the proper use and handling of both sterile and hazardous medications. Compounds and handles sterile and hazardous medications in accordance with established guidelines and regulations
  • Ensures compliance with all FDA, DEA, State Board of Pharmacy, OSHA and other regulatory standards. Follows all department policies and procedures
  • Provides indirect oversight of pharmacy technician and pharmacy support staff
  • Aseptically prepares all sterile admixture requests for patients upon receipt of physician orders from clinical and infusion centers.
  • Assists the Pharmacy Manager in receiving prescription orders for oncology and infusion therapy both for internal and external referrals
  • Maintains patient profile records and prescription files where appropriate.
  • Supervises Pharmacy Technicians and receiving updates on patient problems or problems in the operation of the service.
  • Orders and maintains appropriate stock levels of IV drugs and supplies in compliance with contracts. Assists with the proper accounting and transfer of IV pharmaceuticals to infusion therapy and oncology cost centers.
  • Works well with oncology and infusion center personnel to identify special needs and in drugs and IV medical supplies.
  • Checking and verifying of all medications before they are sent out to be administered.
  • Maintains appropriate logs and cleaning records of biomedical – pumps - equipment. Arranges for preventative maintenance and inspection.
  • Carries out various quality assurance procedures including flow hood cleaning, repairs and inspection, including biological laboratory testing.
  • Assembles supplies and arranges for proper delivery or pickup of home infusion medications and supplies.
  • Maintains the cleanliness of the admixture machinery and keeps IV room properly stocked w/ supplies.
  • Performs special duties and projects as required.
  • Reports to the Pharmacy Manager directly

Specific Part-Time Infusion On-Call Pharmacist requirements:

  • Area: Ability to work in the Phoenix, AZ area.
  • Licensure Requirements: Must have valid, open and active Arizona license.
  • Education: B.S. or Pharm. D.
  • Years of Experience: 2+ years of recent Pharmacy experience within an IV or Home Infusion setting. Recent experience with IVIG and TPN's
  • Schedule: Part-Time, Monday – Friday, between the hours of 12:00p – 5:30p, plus on-call for after Pharmacy hours.
  • Length of Assignment: Temporary - Ongoing
  • Pay: $62.00 / Hour
  • Background/ Drug Requirements: Ability to successfully complete background check and drug screen.
