Top Job of the Month – Long-Term Care Pharmacist in Dallas/Ft. Worth, Texas
Rx relief has an opening for a long-term care pharmacist in the Dallas/Ft. Worth, Texas area. This is a temporary-to-hire position.
The long-term care pharmacist evaluates medication orders from physicians for appropriateness of the drug, dosage, potential interactions, route of administration and stability. They make recommendations for therapy changes to the physician and facility staff if needed, and works with patient medical information to conduct drug utilization review on all prescriptions, as well as reviewing clinical edits to ensure that there is no adverse drug interaction.
Other duties of the job include maintaining a patient database to include appropriate current and past medical history, appropriate clinical and laboratory data and medications. The pharmacist also works with pharmacy technicians to reconcile controlled substance logs with inventory and dispensed prescriptions to comply with state and federal regulations.
The person hired for the job will also work with the chief pharmacist to ensure that cost containment practices are carried out with all dispensing and procurement activities. In addition, the pharmacist provides clinical expertise on drug-related issues for technicians and nursing staff, as well as directors of nursing, administrators and customers.
The job also requires collaboration with the healthcare team to provide clinical and technical support, documentation in the medical record of all communication with physicians, nurses, other healthcare team members, patients and their families, and documentation as required by federal and state regulatory agencies.
They will also be responsible for implementing policies and procedures for safely obtaining, storing, distributing and using drugs and biologicals, as well as disposing of them. And the pharmacist assists staff with procedures for safe and effective methods of obtaining, maintaining, administering and disposing of drugs.
The requirements for the job include a valid, open and active Texas license, an R.Ph or PharmD and at least one year of experience working in a long-term care setting.
